in High School. Must be a member of a Youth league in the St Joseph, Missouri,
USBC Association.
2. Please TYPE or PRINT clearly.
3. Attach following to complete application.
(a) Two Character Reference letters
(b) Grade Average as verified on attached form
4. Give completed application with attachments to your Bowling Center Youth
Coordinator.
5. Deadline for Application is April 5, 2010.
